Ottawa Pathways to Better Health pairs Ottawa County adults who have Medicaid or Medicare and two or more chronic conditions with a community health worker who helps them get to the doctor and dentist, manage their health, and connect to food, housing, transportation and financial help. Call 616-393-5601 (Spanish 616-393-5747) or walk in at the health department on James Street in Holland on Tuesday afternoons or Friday mornings.
